Birth Notice

Within 14 days of the birth (including the day of birth).

*If the city hall is closed on the 14th day, then you may apply until the following day.

The father or mother, person living with the family, the doctor of midwife present at the birth, in that order.

The local authority of the family register, where the family lives or where the child is born.

・ 1 copy of the birth certificate
・ The mother-and-child health record
・ The applicant's personal seal (*in place of a signature)
・ Certificate of National Health Insurance (for those thus insured)

*If the application is late, it is also necessary to apply for a overdue notice.

This registration is necessary for the child to acquire Japanese nationality.

If the parents intend to stay in Japan for 60 days or more after the birth of the child, they must acquire residential status for the child within 30 days of his/her birth, and register it as a foreigner within 60 days of the birth.

If the mother and father have different nationalities, the birth must be reported to both countries.

Death Notice

With 7 days of the death being discovered (including the day of death).

Relatives or other co-habitants, the landlord or manager of the estate.

*Relatives not living with the deceased or guardians may also apply.

The local authority of the family register of the deceased, the location of death or where the applicant is resident.

・ 1 copy of the death ceritifcate or autopsy report
・ The applicant's seal
We will issue a cremation or burial permit at the same time, so please check where you would like to do this in advance.
*There is assistance available for cremation. For more information, please contact the Environment Division.

The Certificate of Alien Registration  of the deceased must be returned to City Hall.

If a cremation or burial is to be conducted in the deceased’s country of nationality or any country other than Japan, that country’s embassy or consulate must be consulted.

Please note that, as a general rule, burial is illegal in Japan. However, some exceptions are made.

Marriage Notice

Valid from the day of issue.

*The marriage notice also requires two witnesses over the age of 20.

The local authority of either of the couple's family registers, or where one of them is resident.

・ Marriage notice form
・ 1 official copy each of the couple's family registers (if not located in Kyotanabe)
・ The husband and wife's personal seals (one a former name)
・ A parental consent in the case of a minor applying

*Your place of residence does not change even if a marriage notice is issused. Those changing their address are required to submit a change of residence form.

The conditions under which married status is established vary according to the nationality of the individuals.

In the case of international marriages, the husband and wife must meet the conditions stipulated by both his and her countries.

Divorce Notice

Valid from the day of issue.

*In the case of divorce by settlement, mediation, admission to claims, arbitration or by decree, please apply for the notice within 10 days of the conclusion or settlement.

*In the case of divorce by consent, the notice also requires two witnesses over the age of 20. However, in the case of divorce by settlement, mediation, admission to claims, arbitration or by decree, the complainant is required to apply for the notice and witnesses are not necessary.

The local authority of the couple's family register, or where the applicant is resident.

・ A divorce notice form

・ A certified copy of the family register (if not located in Kyotanabe city)

・ The husband and wife's personal seals

*In the case of divorce by settlement, mediation or admission to claims, the complainant's personal seal and an official copy of the settlement, mediation or admission to claims form are also necessary.

*Those wishing to continue to use their married name after divorce must apply for a notice acknowledging this within 3 months of the divorce
*If you have children under the age of 20, please apply only after having decided whether the father or the mother will legally hold parental authority.

*There are many other notices, such as for relocating one's family register, acknowledgement of a child out of wedlock or for adoption.
*In order to prevent the issue of notices under false family registers, those who apply for notices of marriage, divorce, adoption, unadoption or acknowledgement of a child born out of wedlock, must also submit their driver's license or some other form of identification. We will notify applicants whose identity cannot be verified of the acceptance of the application in writing.
